  • As soon as you have submitted forms 1 and 2, the NYSF will begin the process to arrange your air travel to the NYSCamp.

  • Please upload a head and shoulders photograph suitable for publication in .jpg format 2 MB or smaller to http://forms.nyscamp.org
• Form 6: Medical Information
  • If you have a life threatening allergy or other medical condition, please call the NYSF immediately!
  • Some portions of this form must be completed and signed by a licensed medical professional.
  • You are required to have a physical examination after January 1, 2019.
  • You are required to be vacinnated against measels.
